Assessing how an individual manages essential daily tasks like dressing, washing, cooking, and mobility.

Care Needs and Support Levels

Evaluating current and future care requirements, from informal help to professional support.

Aids and Equipment

Assessing current and future aids and equipment needs

Long-Term Rehabilitation Recommendations

Outlining therapy needs, goals, and strategies for sustained recovery and independence.
