Since 2021, Jo has worked as a Case Management Assistant, providing invaluable support to the case management team and helping to ensure clients receive high-quality, person-centred care. She works closely with case managers to assist in the coordination of services, communication with clients, families, and professionals, and the day-to-day management of cases.
Jo plays an important role in supporting our case managers by providing both practical and administrative assistance, helping to ensure that care plans are implemented effectively and that clients receive timely and responsive support. Her strong organisational skills, proactive approach, and commitment to delivering excellent outcomes make her a highly valued member of the team.
Prior to joining case management, Jo worked as a Healthcare Assistant with Harrogate and District NHS Foundation Trust (HDFT), where she gained extensive experience in both community and orthopaedic services. This role provided her with a thorough understanding of patient care, rehabilitation, multidisciplinary working, and supporting individuals with a wide range of physical and health-related needs. Her experience and knowledge enable her to provide effective support to both clients and colleagues, contributing to the delivery of high-quality case management services.
